Memo/Criminal/Background Check Process (Memo/Forms to be provided to applicant by hiring supervisor. To be completed by applicant and submitted to State Police  with a $10.00 money order.)

Authority to Release (Signed by applicant and submitted to HR with Verification of Fingerprints form signed by State Police)

Proposed Employee Appointment Form

Citizenship/Visa Status Verified
(Certificate of Naturalization, Resident Alien, H-1 Visa, J-1 Visa, F-1 Visa/I-20/I-94, OPT) (Employment eligibility must be verified before offer of employment)
