Pelican HRA1000 (Blue Cross and Blue Shield of Louisiana)

 

The Pelican HRA1000 offers a low premium in combination with a Health Reimbursement Arrangement employer contribution to create an affordable option for members. Coverage is provided within the Blue Cross nationwide network, as well as out-of-network.

Pelican HSA775 – Active Employees Only (Blue Cross and Blue Shield of Louisiana)
The Pelican HSA775 offers a low premium in combination with a Health Savings Account – which the member and employer contribute to – to create an affordable option for members. Coverage is provided within the Blue Cross nationwide network, as well as out-of-network.

Magnolia Local Plus (Blue Cross and Blue Shield of Louisiana)
The Magnolia Local Plus offers a lower deductible than the Pelican plans in exchange for a higher premium. Coverage is provided within the Blue Cross nationwide network. Out-of-network care is provided in emergencies only.

Magnolia Open Access (Blue Cross and Blue Shield of Louisiana)
The Magnolia Open Access offers moderate deductibles and coinsurance in exchange for a higher premium. Coverage is provided within the Blue Cross nationwide network, as well as out-of-network.

Magnolia Local (Blue Cross and Blue Shield of Louisiana)
The Magnolia Local offers a lower deductible than the Pelican plans in exchange for a slightly higher premium. This is a narrow network (limited providers) plan for members who live in specific coverage areas. Out-of-network care is provided in emergencies only.

Medical Home HMO (Vantage Health Plan)
The Medical Home HMO is a traditional HMO offered by Vantage Health Plan. This plan provides coverage through the Affinity Health Networks and a standard provider network.

Blue Advantage HMO (Medicare Advantage Plan)
This Medicare Advantage plan through Blue Advantage combines medical, hospital and prescription drug coverage in one convenient and affordable plan.

Humana Medicare Employer HMO (Medicare Advantage Plan)
This Medicare Advantage plan offers savings through a network of providers, plus benefits that are easy retirees to understand and use.

Peoples Health HMO-POS (Medicare Advantage Plan)
This Medicare Advantage plan through Peoples Health offers high quality, cost-effective healthcare services to eligible retirees.

Vantage Premium HMO-POS, HMO-POS and Basic HMO-POS (Medicare Advantage Plan)
Vantage Health Plan offers Medicare Advantage plans to eligible retirees as an alternative to traditional Medicare.

Via Benefits Medicare
Via Benefits Medicare provides personalized assistance to help you make informed and confident enrollment decisions and choose the health plan or plans that best fit your medical needs and budget. This option includes a Health Reimbursement Arrangement (HRA).

OGB offers two fully-insured life insurance plans for employees and retirees through The Prudential Insurance Company of America (Prudential). The state pays half of the life insurance premium for covered employees and retirees.

The two plans of life insurance available, along with the corresponding amounts of dependent life insurance offered under each plan, are noted below.

Important Notes

  • Newly hired employees who enroll within 30 days of employment are eligible for life insurance without providing evidence of insurability.
  • Employees who enroll in the life insurance plan after 30 days are required to supply evidence of insurability to the insurer.
  • Plan members currently enrolled who wish to add dependent life coverage for a spouse can do so by providing evidence of insurability. Eligible dependent children can be added without providing evidence of insurability to the insurer.
  • Employee pays 100 percent of dependent life premiums.

Accidental Death and Dismemberment (AD&D)

Who is eligible?

    • Basic and Basic Plus Supplemental Plans
      • Full-Time Employees
      • Eligible Retirees

If retired, coverage for AD&D automatically terminates on January 1 following the covered person’s 70th birthday. If the plan member is still actively employed at age 70, coverage terminates at midnight on the last day of the month in which retirement occurs.

Plan Changes at Age 65 and 70

Plan members enrolled in life insurance coverage will automatically have a 25 percent reduction in life coverage on January 1 following their 65th birthday. Another automatic 25 percent reduction in coverage will take effect on January 1 following their 70th birthday. Premium rates will be reduced accordingly.
Continued Coverage for Dependent Children
A covered child under age 26 who is or becomes incapable of self-sustaining employment is eligible to continue coverage as an overage dependent if OGB receives required medical documents verifying his or her incapacity before he or she reaches age 26. The definition of incapacity has been broadened to include mental and physical incapacity.


This coverage is not health insurance coverage (often referred to as “Major Medical Coverage”). This type of plan is NOT considered “minimum essential coverage” under the Affordable Care Act and therefore does NOT satisfy the individual mandate that you have health insurance coverage. If you do not have other health insurance coverage, you may be subject to a federal tax penalty.

This policy provides ACCIDENT insurance only. It does NOT provide basic hospital, basic medical, or major medical insurance as defined by the New York State Department of Financial Services.
